We work with clients on these types of issues, with Formalization of Authority as key (someone at the very top declaring to the organization that a particular person is in charge). I totally agree with you. It is vital that someone is given responsibility for the running of a project or website. Even if they are not the final decision maker, at least it is them who has to report to the board. We have found this even more important in large corporations where there may be sign off hierarchy. However all too often no one wants to take ownership and decision by comity is never fun.
